If there is an insurance that we are NOT participating with, we will require a $250 deposit/retainer at the time of treatment and will submit all forms. Once the insurance makes their payment to us we will balance bill or refund the patient the difference.

It is a strict policy that we DO NOT offer in-house payment plans. Please understand that your patient;s co-pay or out of pocket expenses will be due before treatment is completed.
